Referring your patient to HiCare is simple
Submit Referral
Complete the referral form below or submit the downloaded form via your practice software. Our clinical team reviews the referral and assesses suitability in line with TGA guidance.
Patient contact & booking
We'll reach out to your patient within one business day to help them book a 30-minute consultation with an AHPRA-registered doctor.
Clinical assessment & treatment
Your patient attends a telehealth consultation. If appropriate, a treatment plan is prescribed and medication is sent via secure, express delivery.
Ongoing support & shared care
Patients receive unlimited nurse support for dosing, titration and follow-up. Referring partners receive correspondence, with patient consent, supporting collaborative care.
